By accessing or using the SIVAYA Health website and services, you agree to be bound by these Terms & Conditions. If you do not agree with any part of these terms, you may not access or use our services. These terms apply to all visitors, patients, and others who access or use our services.

Telehealth Services

SIVAYA Health provides telehealth medical weight loss services to patients in California. By using our telehealth services, you acknowledge and agree that:

• You must be located in the state of California at the time of your telehealth visit.
• Telehealth services may not be appropriate for all medical conditions.
• You have the right to refuse or discontinue telehealth services at any time.
• Technical issues may occasionally interrupt or prevent the delivery of services.
• In-person follow-up may be recommended depending on your clinical needs.

Patient Responsibilities

As a patient of SIVAYA Health, you agree to:

• Provide accurate, complete, and up-to-date health information
• Disclose all current medications, supplements, and health conditions
• Follow your treatment plan as directed by your provider
• Notify us promptly of any adverse reactions or changes in your health
• Pay applicable fees for services rendered
• Not share your account credentials with others

Prescription & Medication Policy

Prescriptions are provided solely at the discretion of our licensed medical providers based on clinical evaluation. SIVAYA Health does not guarantee that any specific medication will be prescribed. Prescriptions are issued in accordance with applicable California and federal laws. Medications are shipped directly to your home through licensed pharmacy partners.

Payment & Refund Policy

Payment is due at the time of service. We accept major credit cards and other payment methods as displayed during checkout. Please note:

• SIVAYA Health does not currently accept insurance for weight loss services.
• Consultation fees are non-refundable once a clinical evaluation has been completed.
• Medication refunds are subject to the policies of the dispensing pharmacy.
• Cancellations must be made at least 24 hours before a scheduled appointment.

Intellectual Property

All content on the SIVAYA Health website, including text, graphics, logos, images, and software, is the property of SIVAYA Health and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works without our express written permission.

Limitation of Liability

To the fullest extent permitted by applicable law, SIVAYA Health shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, or goodwill, arising out of or in connection with your use of our services.

Governing Law

These Terms & Conditions shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law provisions. Any disputes arising under these terms shall be subject to the exclusive jurisdiction of the courts located in Los Angeles County, California.

We reserve the right to modify these Terms & Conditions at any time. Changes will be effective immediately upon posting to this page. Your continued use of our services after any changes constitutes your acceptance of the updated terms.
